The following instructions and forms are for anyone applying / reapplying to the Juvenile Justice Commission as a commissioner or a volunteer. Forms should be mailed to the Juvenile Justice Commission Office.

Print And Complete Forms
Print a copy of both the Application and Supplemental Questionnaire if your are applying and just the Reapplication if you are reapplying. Fill out each form with the required information, and “type” to make sure your writing is legible. If additional space is needed for your responses to the questions in the Supplemental Questionnaire, please write the question number and your answer on a separate sheet of paper and attach.

Mail Forms To The Juvenile Justice Commission Office
Upon completing the forms, mail the following to the Juvenile Justice Commission Office:
- your application
- questionnaire responses
- a resume
- three references

Schedule An Interview
The Commission along with the Presiding Juvenile Judge will review your application. If after reviewing your application you are deemed to be a suitable candidate, you will be contacted and scheduled for an interview.

Get Fingerprinted
If selected, you will be required to pay for a background check. See attached document entitled “LiveScan Procedures for Juvenile Justice Commission Volunteers” for directives.
